Red, Itchy or Sticky Eyes: Which Eye Drops Are Safe Without a Prescription in India, and Which Aren’t

Eye drops for red eyes in India: which are safe without a prescription, why steroid and antibiotic drops need a doctor, and when a red eye is urgent.

Key takeaways

  • The safest eye drops for red eyes without a prescription are lubricating drops (artificial tears).
  • Redness-relief drops can cause rebound redness. Use them for 72 hours at most.
  • In India, steroid and antibiotic eye drops are prescription-only. Steroids can raise eye pressure silently.

In a rural Indian study, 240 of 1,423 young people with a severe eye allergy used steroid drops without a prescription. Forty-seven developed steroid-induced glaucoma.

The short answer: for eye drops for red eyes, start with lubricating drops, rinsing and a cold compress. Never use steroid or antibiotic drops unless an eye doctor has examined you.

1. The Science: Why Eyes Turn Red, and What Steroids Do

The white of the eye is covered by a thin membrane laced with tiny blood vessels. Smoke, allergens and germs all make those vessels widen.

The pattern gives clues. A JAMA systematic review found itching was the most consistent sign of allergy. Lids stuck together on waking, with little itching, pointed to bacteria. Viral infection is the most common cause and usually needs no specific treatment.

Steroid drops calm inflammation fast. But in some people they slow fluid drainage through the eye’s filter, the trabecular meshwork. Pressure builds, often within weeks and with few symptoms. A PGIMER review cites classic data: about 5% of people are high responders and 35% intermediate. Steroids can also cause cataract. That is why steroid eye drops for red eyes belong under an eye doctor’s care.

2. Eye Drops for Red Eyes in India: Read the Label

India’s Drugs and Cosmetics Rules place eye drops containing antibiotics or steroids under Schedule H. Their labels must show Rx at the top left and say they are sold only on a doctor’s prescription.

Before you buy eye drops for red eyes, check the label for these:

  • Steroid names often end in -sone or -olone, such as dexamethasone or prednisolone. Loteprednol is also a steroid.
  • Redness-relief drops usually contain a decongestant such as naphazoline or tetrahydrozoline. The AAO warns of rebound redness and says people with narrow-angle glaucoma should avoid them.
  • Anti-allergy drops relieve itching, says the JAMA review. Check the label for Rx.

At a Pune glaucoma clinic, 4.1% of glaucoma patients had steroid-induced glaucoma. Dexamethasone drops were the most common cause.

Did you know? Doctors first reported that steroids raise eye pressure in 1950, in people taking steroid medicine by mouth.

3. Safe Eye Drops for Red Eyes This Diwali

  • Rinse and soothe: wash your hands, rinse closed lids and use a cold compress.
  • Lubricate: choose preservative-free drops if you need them often. See our lubricating drops guide.
  • Do not rub, and skip contact lenses until your eyes feel normal.

Smoke irritation usually settles indoors; see pollution eye irritation. A firecracker injury is different. In a 2026 Indian study of 67 Diwali eye injuries, 63% were children and over 40% arrived with very poor vision. Do not rub the eye. Go straight to an eye emergency.

Red-eye plan: soothe with rinsing, cold compress and lubricating drops; check every bottle for Rx and steroid names; get a same-day eye check for pain, light sensitivity, blurred vision, contact lens wear or any firecracker injury.
Choosing eye drops for red eyes: a Meerawell summary.

When to see an eye doctor

Get urgent care for eye pain, light sensitivity, blurred vision, a very red eye or any injury (NHS). For a chemical splash, rinse with water for 20 minutes first. See a doctor for red eyes with contact lenses or in a newborn.

Frequently asked questions

What are the best eye drops for red eyes and itching?

Lubricating drops are the safest start. If itching dominates, anti-allergy drops are the usual next step; check whether the label says Rx. Use redness-relief drops only briefly.

Can I use antibiotic eye drops for conjunctivitis without a doctor?

No. In India they are prescription-only. Most infectious conjunctivitis is viral, and antibiotics do not help viruses. Bacterial cases often settle in one to two weeks.

What are the side effects of steroid eye drops?

They can raise eye pressure without symptoms, damaging the optic nerve. They can cause cataract and worsen some infections, including fungal ones. Use them only with an eye doctor.
